When did it become common to carry a dagger in addition to a sword? The few sources I own point to the mid 1200s.
When did it become common for a dagger to be made "en suite" with a sword?

Well, AFAIK, the use of daggers by knightly persons can be traced back to the 1200's (see the c. 1250 Maciejowski Bible for illustratons of knights using daggers). The use of rapiers and daggers made en suite came into fashion around 1520, becoming less common after 1635.
