I know that the sling has been used in hunting and combat. But I'm curious on how accurate could a person be with a sling and could a skilled "Slingsman" kill a grown man with a single well aimed throw?

Yes I killed jack rabbit with it. It removed the middle leaving hind legs and head about two feet apart. I was also a pitcher in college.

I've read that one of the advantages to slinging is being able to use a shield at the same time, but how often was this actually done?

Well it looks like it wasn't unheard. Sorry about the size.

Is that supposed to be a buckler or center gripped round shield the man using the sling is using? Seems rather big for a buckler.
Although a lot of manuscript images of the sling are David and Goliath based, not all of them are.

Can the shepherd's sling be lethal on one throw?
A shepherd's sling can be lethal on one throw depending on the size of the stone, I believe.
